You actually already have all the information you need. It's a 0-4v sensor that goes from 10:1 to 17:1
Therfore, (4volts-0volts)/(17:1-10:1) = 0.57143
That becomes your slope of the line then you add in the offset of 10:1
(Input/0.57143) + 10 =Output
