T42 - 4t65 - Transmission not downshifting into 1st randomly

Has anyone experienced issues where a T42 ECM will not downshift into 1st gear randomly ?

For example, the ECM is programmed to downshift into 1st gear at any time below 41mph. You can do 10 runs, and floor it at 20mph, and it'll downshift into 1st no problem, but the 11th time, it will not downshift into 1st, it'll stay in 2nd gear.

I've attached a datalog where you can see this in the 2nd to the last wide open throttle pull.

Before anyone say's that it has something to do with the shift scheduling, it's not.

I've even set the downshift 2-1 mph all the way down to 50% throttle percentage just incase it was some weird throttle issue.
I've also set all the other pattern tables to the same as the normal table assuming maybe it's randomly referencing the other tables.

I'm thinking this is some sort of glitch.
