Something is wrong if it will only take 15 degrees of timing. Did you try any Torco or Octanium to see if it's crap fuel? Fwiw I would shoot for .88 and a SOI of 360. You are going to see power fall...
Peak torque limit. Your Torque model is increased quite a bit and is likely exceeding the peak torque limit table as Jason mentioned.
Looks like you are hitting torque limits. Also, your lowside is dropping too low and you are commanding too lean of AFR for pump gas.

You don't set the base timing to zero degrees. You need to set the cam timing table to zero degrees, big difference. Sounds like you need to pay someone reputable for a base tune.
Setting Upshift torque management to NONE doesn't seem to be a big deal on A8's. The ECU still pulls timing on the shift. Disabling Upshift "torque reduction" is what will shorten the transmission...

Ime it does nothing on the 6th gens with A8's.
You can select 1-3bar scaling in the VVE editor. then the spark tables axis need rescaled to go up to 2.0.
As mentioned above not much to gain on the engine side with those mods! There would be better gains from transmission tuning. The stock TCM tuning on the A8 leaves a lot to be desired imo.
set virtual sensor to disabled, then go into the stoich table shown in picture above, set it to 14.67 for E0 across the entire table or 14.1 for E10. If you want to run the E15 88 octane set to 13.8....

Needs trans tune as mentioned above. All the A8's I have tuned make the 4-5 similar to the 3-4.
I was thinking corrupted file also. Had it happen before. Make sure to write entire with the new file tuner sends.
Check plugs and make sure one isn't cracked?
I think VVT and VTT is being confused here. More than likely the Torque Model is off. Timing is probably swinging down into the negative range making it surge. Log idle timing and torque and see...

I have been running the same set of LTR7ix in my Whipple SS since 2019 with no issues. I doubt the plug heat range is the issue.
