Thanks for the responses. The car is N/A and the MAF is removed and disabled. Fueling is tuned purely off VVE.
I took your advice and smoothed out the VVE and made the transitions between columns...

The fuel trims show it's actually rich (removing fuel). I don't think it's the VVE as I've tried different fuel amounts and I've tried running it in open loop too.

I have a similar cam. For the startup issue, it appears there is too much fuel for that cam. Try 80 in the cranking VE. Also 1100-1200 VE at idle seems excessive, try lowering that area by 10%
