There was a TSB or recall related to cars shutting off from bumping the keys, or something of that nature. Maybe worn key cylinder?

Those high minimum timing values are not helping it stabilize. But it's also a poorly scaled tune. Well, not even scaled, just lower injector flow numbers. Lots to fix for it to be right.
Raise the VE Maximum table under dynamic airflow. It's 20+% lower than you're GMVE values.
Looks like a possible ground issue with the O2 sensor.
Injector data is a mess, but I curious if something else is going on...
Do you have a log with injector pulse width? That would help see if it's following commanded.

The ground is solid?
Checked voltage to positive, and to ground. Positive voltage was 14.5-14.6v the whole run. To the ground showed 0.02 to 0.1v over the run.
I checked resistance in the wire to its connection points. All between 0.1 and 0.2 ohms.
The ground is solid. Just checked.
