I was under the impression that to be mafless you need to fail the MAF. I feel you need P0101 P0102 and P0103 set to MIL on first error otherwise its still looking for the maf sensor
As 5FDP and Blindsquirrel have said all of your VE tables are completely blank, they need data.
In -engine diagnostics- airflow- max out the P0068 enable rpm table and set the min ECT to 125C for P0101, P0106 and P0121

No. It is showing stoich for the type of fuel you are running and stays constant. The PE table is where you can change your targeted AFR.
Your cylinder volume has your engine as a 5.7L not sure if this affects much but probably worth correcting.
That's the plan, it's how mine is set up.
In the Engine Diag tab have you tried changing the min ECT to 257F ?
I thought P0101, P0102 and P0103 were meant to be set to mil on first error to put the ecu into SD.
A PE of 1.425 is too high, gives a target a/f/r of 10.2.
Could the injector wiring be crossed?
Your cylinder volume is wrong for the increase in engine size, but what is going on with the O2's. One bank adding fuel while the other is taking is a left to right wiring issue I would guess.
Not sure if it will help solve this but I would make the PE ramp in rate at least 1.0 and ramp out drop back to 0.01
