You might want to find a copy of the diagram and make sure the pin is in the right place in the connector. If GM can manage to screw it up, I'm sure Painless can too.
If by change you mean edit it, yes. Or, if you decide, you can go through the sticky at the top of the page for a different config file.
Definitely seeing this problem as well. It seems like it happens more often when messing with the VCM controls.

Not sure if you got rid of your cats or not, but make sure that you don't have the front O2s plugged into the rear wires.
