You guys should read through the transient fuel sticky at the top of the forum it helps alot for throttle transition tuning for rich/lean spikes. It's under the how to sticky.

Also make sure you 0 out the egr airflow table while it's enabled. I've found this will add in airflow and skew your tuning.

Likely the reason you are staying rich is the rapid throttle transition and size of your injectors. You're hitting the minimum pulse of the 65lb injectors and still getting to much fuel. Try this out...

How is the sensor mounted? It needs to be angled with the head down somewhere around 15 degrees at least to work best.

Is it a truck maf with the built in IAT sensor? If it is plug it back in you need the IAT and fail it like 5FDP said.

Those spark tables are pretty choppy in the 395 tune if it doesn't work for you, you can try this one.
