1) why is your idle so high? Seems to be in the 3k+ RPM range?
2). Your fuel trims are suggesting MAF and/or VE tables are way off as it's commanding 8-18% more fuel.
3) Bank 1 and Bank 2 fuel trims are not even, suggesting something else may physically be wrong. B1 seems to be demanding more fuel and B2 is demanding less.
I would first make sure there are no header leaks as that will cause a big issue. If that doesn't resolve anything you may need to check all plugs are firings, all injectors firing, make sure rockers/lifters aren't broken, possibly do a compression test for all cylinders, etc...
