Surge in Open Loop when Cold

I have my cam (226/230 115 ) pretty well dialed in when the car is warm, but when I first start driving in the mornings (temps in the 30*s) I get surge or bucking until the coolant temp warms up to around 120 and goes into closed loop. The STRIMs are showing -5 to -12 and I've tried leaning it out using the Open Loop Gas table (IVT v. ECT temp) and IVT gain table (IVT v MAP), but it hardly seems to dent the reported STRIMs, and it doesn't feel much better.

I could use a few tips for fixing this using my narrow band sensors as I no longer have access to a wideband. Tune is in pure MAF mode.

Cold idle isn't too bad so its just when driving.
