The code is set by your coolant sensor signal fluctuating erratically. Since it's essentially a thermometer, it shouldn't do that. The signal reporting in should 'move' slowly.
You likely have a defective sensor or a faulty connection, etc....
Poll the sensor at the highest rate possible and see if that's the case. If you still have the old one, connect it and compare readings. Investigate and solve your issue!
