The instructions are clearly listed on the upgrade page
It doesn't have to be perfect, it just needs to be done in two weeks...

Those instructions should be listed on the order confirmation page. The way you have it setup right now is confusing

You are responsible for shipping it to us.

Might be a good idea to send users this information once they place the order.
Alot of places make you wait for a confirmation or something to instruct people where to send things (at least that's the norm online)
Just a suggestion to help out take it or leave it
