Hesitation / Stumbling - Low RPM

I've been trying to figure out a low rpm stumbling feeling. I can replicate it by cruising at 2000rpm in 2nd gear. It feels like it is just having trouble. When driving normal (like the cops are around) from a stop to around 2500rpm it feels like it hesitates and after that rpm, it feels healthy and kind of pushes you in the seat. Hard to explain.

This all started after a small second gear pull. Something felt off after that pull and I just can't find out why. It even seems like my mpg has dropped after the fact.

I've attached a log while driving in the areas of the hesitation feeling, and was hoping someone can point me in the right direction.
