Starting to tune my 04 lb7 with 135,000 miles and noticed the reported balancing rates seem problematic. Cylinder 7 shows + 9.5mm while coasting. injectors have 20k on them. What could be the issue? head gasket?

LB7 injectors are known problematic but coasting is not typically the check point. +/- 4 in park, +/- 6 in gear at idle. This is not end all be all, but a guideline check for injector health. You should also google the the return check to see if the injectors are returning excess fuel.

That is a bad injector.
Your only option is replacing the one or doing all 8 as a preventative measure, and cost savings on labor if another fails
