What issue(s) cause REP on P59?

I'm tuning a truck and for no reason it goes into REP.
Doesn't matter when, how, speed, just randomly it'll REP.
I'm not seeing anything blatant on the log, but, google research seems to point
towards wiring or bad throttle body and or tac module.

    Any suggestions?

The log attached is driving into a parking lot. I was circling around looking for a place to park to upload the data logged.
It goes into REP and I'm 13 mph i think, until i get into a parking spot and then idle for a few before saving and turning off.
You can tell REP by the idle speed it goes to 400 something.

It should have a code when it does this. Find out the code number.

There is a GM TSB for multiple year models and vehicles where the conductors in the factory harness break 6-8" from the throttle body connector causing many different codes and REP. I would read the codes and start there first. If it is a factory GM truck or using a modified factory truck harness, search for the TSB for that model as well, I bet it falls under it. I have put new pigtails on probably 15-20 GM trucks over the past 20 years specifically for this problem. I prefer open barrel crimps that fit over the wire insulation for strain relief and 4:1 marine heat shrink with adhesive to seal out moisture for repairing sensor wires.
